L'histoire que nous présente NAOKI URASAWA ( 20th century Boys ) est celle d'un docteur japonais travaillant dans un hôpital en allemagne en 1985, le docteur Kenzo Tenma.
Un jour il reçoit en urgence le maire de la ville et un petit garçon menacé de mort suite à une balle dans la terre, il décide d'opérer le petit garçon, dont les parents ( des diplomates étrangers ) ont été retrouvés morts, et sa petite soeur, amenée en état de choc.
Il sauve le garçon en dépit du maire, et se voit retrogradé comme simple chirurgien.
Quelques jours plus tard, le directeur et 2 autres hautes personnes de l'hôpital.
Les 2 enfants ont disparus sans laisser de trace, Tenma se fait nommer chef de personnel.
Le commisaire Runge, du BKA ( police fédérale allemande ), est chargé de faire la lumière sur l'affaire, mais piétine par manque d'indices clairs.

1995, des faits simillaire se produise dans un autre hôpital, auquel Tenma s'était rendu suite à une demande du personnel pour une opération particulièrement difficile.
Un patient s'échappe, Tenma lui court après, et découvre une horrible vérité:
Le jeune garçon qu'il avait sauvé 10 ans plus tôt, est non seulement vivant, mais est devenu une chose impossible à décrire rationnellement, celui que l'on nommera Monster ! Il tua le patient et se dévoile à Tenma comme étant celui qu'il a sauvé il y a 10 ans, connu alors sous le nom de "Johann (Liebert)".

Qu'est devenue sa soeur ? Qui est-il ?
L'intrigue ne fait que commencer, l'auteur nous plonge dans une Allemagne corrompue
où agir seul est recommandé, de nombreux personnages viendront se mêler à l'histoire, tous mêlés de près ou de loin à l'existence de Monster, mais au fond, que cache son existence ? ... à vous de le découvrir ;)

Ames sensibles s'abstenir, ce n'est pas un simple manga, il y a un fond de vérité dans ce que nous révèle l'histoire. Ce n'est pas réellement un manga d'horreur, mais ce que révèle l'histoire peut le faire penser sincèrement!!
